· Degenerative disc disease (as defined by neck pain of discogenic origin with degeneration disc confirmed by patient history and radiographic studies 
· Spondylolisthesis 

· Trauma (including fractures, dislocation) 

· Spinal stenosis 

· Tumors 

· Deformity (defined as scoliosis, kyphosis, or lordosis) 

· Pseudoarthrosis 

· Failed previous fusion
