INDICATIONS FOR USE

· Spondylolisthesis (Grade 3 and 4)

· Degenerative spondylolisthesis with objective evidence of neurological impairment

· Trauma (including fractures, dislocation)

· Spinal stenosis

· Deformities or curvatures (defined as scoliosis, kyphosis or lordosis)

· Tumor

· Pseudoarthrosis

· Failed previous fusion
